The quick release never failed but the two piece stap has a buckle on it and it would slip when it got wet. It decided to come apart while I was climbing a dive boat ladder in 5 foot seas with Steel 98s and a stage bottle. I got rid of that hunk of crap and have been happier and safest ever since.
