This is the first opportunity I have had to contribute, so here goes: We use the AP-Valves type D-SMB's all the time, without any problems. Instead of filling the D-SMB by purging the second stage with the mouthpiece facing upwards, I "shake-out" the D-SMB so that it is unfolded, in front and slightly above me and clip it onto my reel. I then tilt my head and exhale into the 'hole' at the bottom. From 90 feet it only takes one small puff to ensure the D-SMB reaches the surface fully inflated. Obviously at a shallower depth, you would need a couple of exhalations. This should eliminate and problems with regs freezing and with practise, you should be able to do it while maintaining neutral buoyancy.
