Then add to that that the person ultimately in charge of their budget and operations is probably either 1)not a diver or 2)been out of the water soooooo long that they still have equipment from the days of Sea Quest. That and it can take a long time to run things thru procurement. Hell, for something simple like Mobile Data Terminals in Police Cruisers by the time the money is appropriated the bids solicited and the equipment installed, the MDT's are often out of date. As to the mounting upside down, I saw a documentary with Police / Rescue divers in OZ with their cylinders (in one case, a single cylinder) mounted upside down. While it looked sorta neat (i.e., unusual), I could see all kinds of problems with it but since they were already mentioned, I won't bring em up again. I'll stick with my "convoluted" OMS wings but I'll keep my manifold doubles upright where they belong.
