Bob,

In American Cave diving, decom gases are stowed/stashed at the appropriate
depths in the beginning of the cave (or where ever the stops will occur) -
unless the decom is short enough not to require extra staged deco gases
(ie, short penetrations with total deco less than 30 minutes.).

Obviously, the statement about thirds rule allowing gas for exit and decom
is only referring to those short dives where this is feasible.  Again, for
the longer deco's bottles are staged into the cave specifically for this
and calculations made to allow for enough and then a safety reserve of
deco gases.  Obviously, with the longer and longer deco times, multiple
gas switches become the rule, rather than the exception.
