I've got a couple of these (one for each arm :-) for cave diving --- the
left one has compass, writing pad, pencil and knife (after carefully
checking the knife didn't affect the compass), the right one has depth
gauge, shears and search reel. I didn't put the watch or computer on them
because I don't always take them, and anyway I don't use the consoles in
the sea.
You can make them out of various kinds of plastic, bent to shape with hot
water or a small propane torch (easier, but careful!) - something a bit
flexible is usual, but one of mine is rigid and "hinged": ('scuse ASCII
graphics...)

Cross-section ----------
                | * <- stainless key ring hinges
                | |
                | | <- multi-layer "book" of writing slates

I stacked a few sheets of roughened white plastic, hinged together -
perhaps over the top in many cases (you write quite a lot cave diving over
here).

Plan on making *two* - the first one won't be "right" in some ways. I made
the first ones quickly, used them a few times, decided what I *really*
wanted, and then did it properly. A point about attaching a compass --
offset it about 20-30 degrees or so, it makes taking bearings and following
courses easier.
