Most good dry suit underwear for cold water is made of material which will keep you warm even if you are damp. When my old suit was getting past the point of no return, it leaked a lot, but I was never too cold to function, even in the Great Lakes which are typically 37-44 degrees F at depth. These were recreational dives, so long decompression was not an issue. If you are concerned about this you can do what haz-mat divers do, and wear a dry suit over their dry suit. This way you have two layers to penetrate before a leak would be catastrophic. When my suit was a damp suit all of the time, I just wore a wet suit as the insulation underneath. It was plenty warm, even if it got wet.
