It seems that regularly deaths are over pieces of kit that unlike lives are so easily replaceable. This is more regular in non tech diving - people do need to get into the mentality of not taking knee jerk reactions when they loose or drop bits of kit. It is a great shame if this is the case as detailed below as it is a waste of a life.
