This is too bad. It is also the single most common "tech" diving cause
of death. This is why we rig the gear the way we do, and why we mark the
tanks and wear them the way we do - to avoid this . 

 Between this and teh practice of blowing air on top of already weak
trimixes and diving them repeatedly, or the even more insane practice of
diving air on the back and mix in a stage, we can expect many people to
die. 

 I really wish that the agencies were not so grossly incompetent, and it
would also be nice if the more experienced guys up thre took the lead in
doing it right.
