I made the statement that this "sounded like" lack of training and or experience because that is what this sounds like. Most deaths on the Doria occur due to just that. The dive boat Seeker and Wahoo are the most experienced boats up here running trips to the Doria, and I bet the Captains and Crew would tell you the same. Of course, I could be wrong.
