I would think that the dive was beyond the abilities to be expected of the 
student.
Our ( recent ) first mix dives were dives that we had previously done on air. 
 (40-45m, still possibly a bit deep for airaccording to some, but then we did 
not know better at the time).  Anything on mix was an improvement as far as 
narcosis went, we were already used to gas switches).

What I find unusual is that so much was different in this dive.

1) It was Jane's deepest.
2) She may well not have had 2 gas switches before ( I don't know)
3) It was a first dive on mix ( there may have been a slight apprehension)
4) She was allegedly told 'it was every man/woman for themselves'.

My buddy and I found that even though the narcosis was less, there was still an 
awearness of the depth = stress.

Too much was different.
The impression was also given earlier that Jane had 'fast tracked' her 
training.
ie, although she was an experienced diver, she had done most of her 'technical' 
diving over a short period of time, and could not be expected to be totally 
familiar with her equipment and the task loading involved.  As I don't know her 
(or Derek) I can't say otherwise, but I would suggest that if what has been 
printed is true, that she was not ready for this dive.  Both she AND her 
instructor should have realised this.

Well you asked for thoughts.

I must add that all of that is based on what I have read on the list and that 
my only right to comment is that I am at a similar stage in my training to 
Jane.

As far as agencies go, I would say, as I think that I have said in the past, 
that they are irrelevant.  There are good instructors and there are bad 
instructors. We need to get rid of the bad instructors.
