The problem is, it is beginning to look like no one is learning - the
Pompano tragedy is almost a carbon copy of the WPB disaster - though the
instructor's did manage to keep the death toll down, by making no attempt
to rescue, so maybe we cold say that he learned something from WPB.

If the agencies involved had shown any sign they could learn, and maybe
reacted to the WPB deaths by doing an urgent reexamination of their
standards, of the qualifications of their instructors, and by reminding
their instructors of a few common sense type rules that might have averted
both tragedies, then this latest one would not have happened. But they
didn't. That being the case, it is completely appropriate that we come down
on them, and come down on them hard.
