Thanks for your comments.  Though I've never had the problems you 
mention with inflators, I've seen many others that have, often as a 
result from bad handling (dragging gear over sand or small gravel 
beaches) on club or rental gear.  Sand gets in the button's mechanism, 
or up the hose's "O" ring and then the fun begins.

Once I saw a  second stage so full of tiny pebbles on the "wet" side of 
the membrane that it jingled like a  musical instrument.

But these are things that usually happen to beginners who dive from a 
beach and are not aware that sand, small gravel and diving equipment 
don't mix well..  As Jim points out, with normal wear and tear, the 
primary cause should be insufficient maintenance.
