PSI is always quick to jump on any cylinder disaster to promote the notion that only PSI-trained inspectors can and should be inspecting cylinders, but in most of the cases I've followed, ANY inspection, properly done, would have prevented the accident, except for a few "act-of-god" cases where NO inspection would have. And anyhow, the PSI trained inspectors I know all inspect tanks just like the non-PSI trained ones, by (99% of the time) taking a quick peek rather than following the PSI protocol.
