Thomas,  

The Abe Davis Award is available from NSS-CDS.  An application form may
be downloaded from the CDS Webpage--thanks to Robert Laird's quick 
action today (CDS Webmaster).  

The Wakulla Award is presented by NACD.  There are three levels:
Bronze (100 or more hours bottom time or cave dives), Silver (500
or more......) and Gold (1000 or more.......)   

Copies of both applications are available at Ginnie Springs Inc., CDS, 
NACD, or many instructors.  

Of course, cave divers who have not had dpv training should consider
this option available to them.  (BTW, I am not an instructor). Training
and experience are both crucial.

Complete criteria for both awards is available from the agencies.
And to answer another faq:  there is no fee involved in applying for
either award.  One does not have to belong to NACD to apply
for the Wakulla Award.  Membership in NSS-CDS is one criteria for
applying for the Abe Davis Award.
