Used to get them all the time. What solved it for me was to change my 
breathing technique. When I exhale, I let the air go out naturally, then 
at the end, I give an extra push from the diaphragm to empty the lungs. I 
do this about every 3rd breath or so. Soon it becomes automatic.

Another thing to do is on your way to your first stop, halt your ascent 
every 10 or 15 ft. for one breath. This makes sure your sinuses and ears 
are catching up with your lungs.

Another problem I had was with an ill-fitting mask. On the old mask the 
strap had to be tight to keep it from leaking. A properly fitted mask 
uses the strap for positioning only, it should just sit on your face 
without leaking. Same go's for your hood, should not be too tight.

There are many things which can cause headaches and assuming you are 
doing everything else correctly, these might help.
