Yeah, that's why ball valves are not recommended in O2 service...they're quarter-turn valves and it's relatively easy for the clueless or forgetfull to just crank open the handle in a second. I suppose a ball valve would actually be a good valve for O2 - very little pressure drop due to "flow-through" design, and not much of the valve is exposed to direct flow - if it were equipped with a gear-driven actuator to slow down the opening/closing.
