If you had bothered to repost the responses to this post, it would show that the big, #1 error in this situation was the use of the ball valve. As I stated in my original post, a ball valve is a real lousy choice in oxygen and, in my opinion, in any scuba related compressed gas application. Ball valves are designed to A) be cheaply made (no threads required) and B) go from full on to full off in 1/4 turn. Neither feature is desirable when pressures can reach 4000psi.
