No you don't. You just specify the hose with a 1/2 inch connection to the
first stage, if you must keep using the big hole.

I use these regs (Apeks TX50) and I am extremely happy with them. But
there is no need for an adapter. Either do as I said above or, if you
dive doubles or H valve (in which case there are only two low pressure
hoses on each first stage), spin the turret around and use the side with
two 3/8 inch connections. Then you can buy standard hoses for less
money. That's what I do.
