Because of tremendous task loading which can easily overwhelm a diver and
contribute to fatal errors. Also possibly narcosis can be a contributing factor
in some cases, as can other medical conditions (unkown factors) that might not
be found until an autopsy is performed (or never known).  There are simply to
many factors to be able to make any simple universal blanket statements or
completely safe proceedures.
