*Nothing* anyone does will make these units absolutely 100% safe,
whether we're talking deep cave diving expeditions using full CCRs
or standard open circuit recreational scuba.

Try as we might, people will have DCS, some will have embolisms,
and, many other accidents will occur.  Even when the technology
exists to warn divers of impending DCS, some will not heed the
signs or use the tools, and some will use them improperly.

Nothing short of banning diving altogether, and enforcing that ban, 
will prevent every accident.  I'm not advocating this, but I am
advocating *individual responsibility for your actions* - something
that seems to have fallen by the wayside in this litigous society.

While it is often very heart wrenching, anecdotal evidence does very
little to statistically prove whether certain safety measures work.
It's like the tables and DCS issue.  There are some people that have
DCS even when they dive well within the tables - there are others
that break every rule in the book yet never show signs of DCS, yet,
statistically, the tables do work.  It is virtually impossible to correlate 
this phenomenon without studying the exact details of the individual 
circumstances and even then, very little light may be shed on the situation.
