The BSAC produce a full 'incident report' every year, containing brief details of all fatalities, bends etc reported in the UK - either through the BSAC, by the Coastguard, police or even through newspapers. They're all open water air dives - no cave/nitrox/mix, but do have deep (30m+), wreck and deco incidents. The quality of the reports is pretty sketchy - nowhere near the detail of Aquacorps for example, but usually has enough info to let you know what went wrong. Unfortunately, there aren't many reports of incidents where things went wrong and the 'victims' got out through good diving practice, but the way of the world is that they are usually kept as bar room stories rather than accident reports...
