I have worked on the assumption that a neck O ring is most likely to go
after the pillar valve has just been reasembled and next most likely when
the tank has just been/is beeing filled. Both of these are not
life-threatening to the _diver_ and are extremely unlikely. The chances of
a neck O ring blowing whilst underwater I recon to be negligible however
this might change if you use a manifold and that manifold is causing stress.
