Indeed. As one who is interested in expanding my diving to caving (through proper training and certification of course), reports like this are critically interesting. I guess what I mean is that I'd _like_ to hear that the reason was something that was a happenstance, not something inherently wrong with procedure. This is, of course, because I feel safest when my safety is under my control, and not left up to fate. Naturally, one cannot achieve perfection in this goal, but the risk can be acceptably minimized.
