Well, _first_ they took a gas deeper than its MOD. If they hadn't done that, this wouldn't have happened at all. Then one diver made a human error, and his so-called "buddy", who was there for the express purpose of backing up the fallible brain, did not do his job. Those two _should have_ been facing each other at all times during the stage bottle pick up. The "buddy" should have noticed IMMEDIATELY that the diver was about to breathe the wrong bottle, (had it been properly marked) and prevented this from happening. Or were they doing some goofy crap like carrying the bottles with them for the entire dive??????? It is _so_ easy not to kill yourself when you dive. It's when you start breaking simple safety rules and convoluting everything that deaths occur.
