John Lippman has covered it in 600 pages with his book "Deeper Into Diving" I
think its available through DAN. It covers the theory and
derivation of the following tables:
US Navy, Huggins, Bassett, NAUI, New US Navy tables,
Swiss/German, the ZH-L system, Buehlmann Tables,
Buehlmann/Hahn tables, DCIEM, BS-AC, RNPL/BS-AC,
BS-AC '88, and the PADI recreational dive planner.
It's also (for those interested) got some notes on
omitted & interrupted decompression procedures as per:
US Navy, BS-AC, Buehlmann (1986) tables, Buehlmann/Hahn,
and DCIEM.

Yes info on bend rates and the old US Navy bend-o-matic
tables go to 300ft.
