In "Diving an Subaquatic Medicine" Edmonds & co quote a case report of an Australian Navy diver who jumped into 20 ft of water with manifolded 72s. The blowout plug on the reserve side (they leave the tanks isolated, and decant when the one they are breathing from runs dry) had burst sometime earlier, and during the dive, the tank filled with seawater.
