8. Have you used underwater communications systems?  What types?

9. What is your most commonly used dive light?

10. Do you service your own equipment?

11. Are you a dive industry member? (ex. store owner, instructor...)

12. What is an average depth dive for you?

13. What is your current maximum depth dive?

14. What is your current maximum depth AIR dive?

15. What training agencies do you hold cards from?

16. Have you had any technical dive training?   List types?

17. How many years have you been diving?

18. Approximately how many dives have you logged?

19. Have you ever used a rebreather?  What type?

20. Have you ever dived on mixed gas? (excluding nitrox)

21. Have you ever dived using nitrox?

22. Do you use Oxygen for decompression?

23. Do you dive double cylinders?

24. How often do you dive?

25. Have you ever suffered from SEVERE Narcosis?
              At what depth?       On what breathing gas?

26. What skill level do you consider yourself?

27. Do you dive with a dive computer?

28. Have you ever used a PC computer generated dive table system?
              What kind?

29. Have you ever had DCS? (bends, DCI, whatever you want...)

30. Were you diving within the limits of the table used?

31. List the areas of diving that interest you most.
    (ex. deep, photo, wreck, cave ...)

32. Are there any publications you feel are a must for the active
    advanced diver?   What are they?

33. Is there anything the dive industry could do to make your dive
    life just a little better?
