Since 1970, John McAniff, Director of the National Underwater Accident Data
Center (NUADC) at the University of Rhode Island (USA), has published
extensive fatality reports each year on recreational/sport divers.  The
entire collection (or just selected years) can be had by contacting John.
By-the-way, John has been working cooperatively for the past few years with
DAN to produce one report of accidents and fatalities, under DAN's banner.
In the past, PADI, amoung others, has supported the work of both DAN and NUADC.

You may be interested to know that in the US the reporting of accidents and
fatalities to a central organization was, for many years, rather haphazard:
some of the fatality cases on file at NUADC contain only a brief, two
paragraph clipping from a single newspaper, while others contain detailed
reports from eye witnesses, autopsies, statements of the victim's training
and experience levels, etc.  By the nature of the individual fatality
"reports," there is no consistent data included (other than the victim died,
of course!).  This makes conducting an accident analysis challenging, to say
the least.

I worked with John to produce three accident analyses, the results of which
I presented at various dive shows.  Two analyses I wrote up and had
published in various places.  The first analysis was devoted exclusively to
wreck diving fatalities amoung US divers, and covers the years 1970 to 1990.
The second analysis covers training and diving lessons from wreck diving
accident analysis.  In both cases, suggestions were offered for divers to
improve their safety while diving.  In the latter case, I made observations
and suggestions for strengthening diver training in the US at all levels.
