Yes, but complacency kills fits in here , too. Thew Milano accident which killed 9 people was caused by a leaking mask in conjunction with a catalytic pocket stove a patient had brought along. Nobody was checked prior to entering chamber. Things have changed a bit, since then, concerning patient checks, and fire extinguishing systems.
