  Well since the list works again ill try to bring something on:

  Have you ever notice some pictures in some " serious " tech magazines ?
  It makes me wonder how could those divers live to make it over and over
  again ...
  Large steel deco tanks , wet suits and 18ltr on their back , " you name
  it " regulators ....
  and the story goes on and on ....
  I also  really find my self amazed when I saw the Diving fatality report
  for year 2000.
  I was even more disappointed when I went through the reasons that people
  got injured
  Or killed.  I got   so  impressed  by this unique symptom ,  the "   I
  wanna do it my way
  because my way works " approach.. .. And then they die or
  they spend the rest of their life on a wheelchair. Or the other type ,
  the ones that they want
  to re-invent the wheel. Those are the guys that you see every day on
  some diving
  magazines , showing off  in a 70M dive with twin independent  and 15ltr
  steel stage
  tanks ,  breathing air.  It makes you want to  find the editor and ..
  Why the magazines don't show those people when they get treated in
  decompression chambers ?
  It doesn't make sense why some people  don't want to learn from others
  mistakes ?
  Why  don't this " expert " divers get  a copy of those Fatality reports
  every year . ? Why most  organizations give so much
  Attention  on what you have to do rather on what you  SHOULD NEVER  -
  EVER DO.

  And the story goes on and on ....
   people   diving wrong gas , wrong equipment , wrong attitude ?
