It's amazing how things go around full circle. Back in 1973 when I was certified my instructor (who was about 6'3 and over 300#) pinned us to the bottom of the pool and did the "harassment training" portion of the course. He would, in no certain or predictable order, rip off a mask, pull out a regulator, or turn off the air supply. This was to make sure that you were prepared to handle these situations since they WERE going to happen sooner or later and you wouldn't know when or what then either. You also had to pass the exam AND pool skills before you were INVITED to the open water checkout dives. There was only two dives required back then, but we also did have a "skills demonstration" that was a one time through of the mask being pulled off, the regulator being pulled out, and the air being turned off that resulted in having to buddy breathe to the surface that had to be successfully completed for the instructor. This may sound excessive, but it was a GREAT preparation for the things that have actually happened ever since.
