I use a "safety sausage", an inflatable neon tube, and a Dive Alert,
an horn powered from my tanks.

These are required by my club (Staten Island Sport Divers) for club
trips, primarily due to a bad experience by the club coordinator.

In the New York/New Jersey area, these are not uncommon.  I know of no
divers here with flares, though I have heard people considering something
heavier to deal with boats.  Flags are only used by shore divers - 
if you are diving from a boat, the boat shows the flag.
