This actually happened to a instructor i know a couple of years ago. It was
a first aid oxygen cylinder that he always had with him when teaching OW
students. The situation was actually rather funny, he was demonstrating how
to breathe from the life saving cylinder to the students and got gassed
badly. Almost died on the spot and i had to drive him to the hospital.They
injected anabolic steroids and shit in the lungs, he recovered after a few
days.

When analysed by the police they found that it contained chlorine gas.  We
thought that it somehow got contaminated in the swimingpool as we always had
it with us even during pool sessions.  I don't know if this was the case and
the police investigation led to nothing. This particular swimingpool had at
the time problems with the filters so they got bacteria in the water and
therefore had to increase chlorine levels. It smelled chlorine in there.

Was the cylinder in your case a deco cylinder or a first aid cylinder?

I recently became aware of an incident in which a tank of medical grade
oxygen was contaminated with chlorine or a similar substance. The
resulting exposure caused the user severe pulmonary damage. Anybody have
any ideas as to how this type of contamination can occur? How medical
grade oxygen cylinders are cleaned etc.?
