Yes, there have been several in the WKPP and among WKPP divers, but none during a WKPP dive while I have run the Project, and none at GUE. We lost Bill McFaden in 1989 to deep air at Little Dismal, Parker Turner ( former Project Director ) in 1991 to a cave collapse, Sherwood Schile in 1992 to dving when he should not have been, Mc guirre in 1995 to the wrong gas on a dive outside of the Project, and Exley to Rule Number One in 1994. We have bent the piss out of a lot of people who had shunts, but they took themselves out of the game with no further injuries, and we otherwise have drowned some but got them back, bent some badly but got them fixed, and otherwise have not had any problems. Much of our curent thinking is shaped by these events - I can not tell you what it was like to surfce at a sinkhole and tell the team that Parker was dead.
