After following the discussion on blown tank valves on techdiver, I
thought of letting You know how this problem was dealt with here in
Finland

Concerning the mis-screwn valves (hmm..), we had some similar incidents
here in Finland a few years back. Here the case was mostly
cone thread valves screwn into a cylinder thread tank neck. At that time
the state agency responsible for pressure proofing supervision
recommended that both the neck of the tank and the valve should bear
markings of the thread. The diving industry quickly (and I think voluntarily)
accepted this procedure. Since, You couldn't get Your tanks filled or
pressure proofed (every 5 yrs. in here) if they didn't have the
thread engraved both to tank neck and valve. The pressure proofing
firms also inspect the threads and make the markings if needed. After
that, no more blown valves.

Good diving to You all for 1995,

Y. T., Juha
