Interesting how you answer your own question, accurately I might add, while 
at the same time suggesting that these could not possibly be the right 
answers.  Habit is a powerful force in human experience.  Ask any junk food 
eater.  

But perhaps there is another explanation.  Maybe once someone is both 
properly trained and properly configured they are too terrified to dive off a 
Jersey dive boat and move away.
