I think that thing uses a fixed orifice in a sealed chamber connected to a small air-filled bladder. Compression of the bladder moves air into the chamber and decompression moves air back to the bladder. The size of the bladder is indicated on the meter with calibration to indicate decompression information. It's certainly not a good approximation of actual tissue gas-loading.
