Apollo 1, named as such in memoriam to the crew. An O2 hose severed in the command module during a test launch. Apollo 13, a short in one of the O2 stirrer circuits blew the tank open, along with much of the water and therefore power from the H2 cells used. I was told on techdiver a while back that the 40% limit for cleaning was based on work done after the Apollo 13 accident, but I have no better reference. Jason.
