In all 5 cases of detrimental IWR that I am aware of, none followed proper
procedures, and all were doing it on air. In 2 cases (the 2 divers off
Sussex), nobody knows what happened. In another case, a person who had
pain-only symptoms prior to IWR "fixed" those symptoms with IWR, but then
later on developed neurological symptoms (would he have got the
neurological sooner had he not tried IWR? No way to know, but for the
sake of conservatism, I'm classifying this as a detrimental attempt at
IWR). Details are very sketchy of the other two cases, but they appear to
be the only real examples backing up the "if you try IWR on air you will
only add more nitrogen and make things worse" theory against IWR (compare
that with over 500 cases of air-only IWR by people who didn't know what
they were doing, who successfully reverted symptoms ranging from sore
shoulders to unconsciousness/paralysis)

I'm only aware of a handful of cases involving the use of O2 underwater
(i.e., proper protocol), all successful, none involving o2 toxicity
problems. Proper protocol mandates the use of a full-face mask. I'm not
sure what I would do in the event of a seizure...probably (almost
certainly) abort IWR (this is why having a tender could be very important,
which might seem contrary to my previous assertion that the tender may be
optional). None of the published methods recommend anything in particular.
