Check to make sure that the gas he was switching to was the correct MOD?? That is SOP for all the deco I do...we always do switches one at a time, and watch to ensure that my buddy has grabbed the correct bottle. That is a backup to the correct procedure which is to leave the reg off until you are going to it, reach down, turn on the correct stage for the depth, run the reg over the shoulder...if it works, gee whiz it's the right reg, if not, re-park it and grab the other reg. This is not brain surgery, just good training.
