Subject: SECOND STAGE FAILURE

During our decompression stops after diving the "Chester Poling" off of
Gloucester MA. this weekend my partner experienced massive second stage
failure. He was diving a Sherwood Oasis approximatley 10 years old.
While decompressing at 10 feet his second stage blew off at the the
point where the hose meets the connection. Has anyone experienced this
with any other type of gear? Both divers safely made it to the surface
without incident. Thanks.
