The ScubaPro / Soniform FAX number is (800) 213-0740.
Soniform and ScubaPro are both subsidiaries of JWA (Johnson Worldwide 
Associates).  Soniform is located with ScubaPro and Uwatech (also a JWA 
company) in El Cajon, CA.  Soniform is the major manufacturer of OEM BCD's in 
the US.  They, of course, also make all of the ScubaPro BC's.
