Early last year I lost a camera ($200 variety, not a Nikonos) at Devil's Den. I waited a couple of weeks to see if some one might find it and turn it in. Then, I called a claim to D.E.E.P. They took some information over the phone, faxed me a form to sign and I had a new camera a week later. This year, despite the claim, my premium was slightly lower. Note that they send you replacements. They do not give you money to buy your own. The premium is based on the total value of the equipment you insure and you establish the value when you take out the policy. Therefore, it behooves you to set a reasonable but low value (e.g. best mail order price) on your gear. If it is lost or stolen you are going to get an exact replacement anyway.
