How do you figure the O-ring came flying out? There's I just experienced a tank O-ring failure 3 weeks ago. This tank was hydro'ed this year as well as vip'ed from a local shop. The O-ring, don't ask me how, came creeping out of its seat about 1/3 of the way. This is the first time I used the tank since inspection. It happened on my rental boat in Big Pine and not in my suburban journing down there from NJ. Thank goodness for that 'cause we had 8 tanks and 4 ponies (did I say that here) loaded in the truck and finding it would have required unloading it. Not to mention the noise that continued for 20 minutes or so.I've saved the O-ring to break the shop owners stones. In looking at it (in front of me now) I notice that it split length wise but you could hardley tell unless you bend it somewhat. Looks to me like a defect. Never thought it'd happen to me. Getting back to the point of ponies...(and I am in the process of DIR, I just placed an order last night). We were (my wife and son) on our way to a new wreck "Ocean Alley" via the rental boat in fla. I have a 90cf for my first dive, my wife uses her short fat 80 and this tank in question would have been my son's. It was his first deep wreck dive, we were leaving a dead boat moored above and me being the nervous fatherly type with detailed dive plans was more than stressed out. My son thinks he knows it all (16) and when we go over our plan he listens to me like ....well some of you know. Anyhow...If this happened under water having a pony (or 3 for that matter) would've been less risky in our abort than not.
