This IS EXACTLY a failure in dive planning and gas management. Why would I be ANYWHERE but on my way back to the hook at 1500 psi? With a 3000 psi cylinder, that's HALF, and with a 2640 is HALF + 200. Where is your turn point for non-penetration no-deco dives? You can construct many of these kinds of scenario.
