In Europe (at least in France, Switzerland (not to be confused with Sweden) and Italy, cave divers don't use short and long hoses and there are no primary nor secundary regulators. We dive with at least two *equivalent* regulators, each with a standard length hose. Every 10 minutes or so we switch to the next regulator. We also only use completely independent tanks. Cave dives here are usually done solo, sometimes 2 divers, but even if we dive with a buddy, we consider the dive as a solo dive (everybody is responsible for himself). Running out of air is considered totally inacceptable by us. Freeflow is almost impossible (we use Poseidon regulators, never seen one freeflow even in very cold water (close to 32 F)).
