There is no standard.

Several agencies around the world have attempted to set "standards", and 
the resulting mess is an assortment of "standards", which is, by 
definition, no standard at all.

Boat "captains", dive resorts (notably the now- defunct Mako Lodge in 
B.C.,) and dive shops all routinely ignore tank markings.
