Thank you for your excellent and informative post.

I have a few more questions, if you have the information, or can provide
me with contact information for other sources.

Which regulator was he breathing from when found?  The 80/20 regulator
would suggest an O2 hit around 70 feet, etc.

Was it this divers routine procedure to dive with the isolation manifold
isolated?  We would presumably need buddies, and/or mates on the Seeker,
to answer this.

By any chance, was the right hand cylinder's residual contents analyzed
prior to mixing it with the left hand's?

BTW, I do not want to hear any criticism from anyone about performing
basic observations on the diver's gear.  This death was well outside
Suffolk County waters (three mile limit, I believe), and did not fall
under the jurisdiction of the Suffolk County Medical Examiner until the
remains are landed.  Sometimes Andrea Doria expeditions head for Block
Island instead anyway.

What is more, unless you have photographic evidence of the Suffolk
County ME with three prostitutes and a goat, do not even bother trying
to get the facts there.  They perform an prompt autopsy (which of course
normally yields no information, unless death was by MI); and issue a
preliminary death certificate that says death by drowning.  Then they
send the gear off to a secret contractor, who performs secret testing.

Then they issue a final death certificate, that says death by drowning.

To the best of my knowledge, the only interested party who normally gets
to read the ME's report, is in fact the deceased.

The deceased's family, if they know enough to demand that report, are
unlikely to have the background in diving procedures to understand it. 
(By the way, I just do
not have the stomach to bother the families for it.  The SCME refuses to
release the name of the person making arrangements for the family...)

DAN does not request the ME's report.  All accident reports to DAN are
by voluntary submission.  I know that e.g. the local chamber routinely
submits reports to DAN, but I have no idea whether the SCME does, or
whether ME's in general do.  The captains of either the Seeker or Sea
Dweller certainly may, but that would not include full equipment
analysis, or autopsy.

Does anyone have other experiences in other jurisdictions?  Animal
photos (I guess for  four reports, I would need four photos!)
