If all those instructors and instructor trainers and training directors died because their training let them down, we have to worry about their training organisations. If, on the other hand, they died by their own stupidity, then they should never have been instructors, instructor trainers or training directors in the first place, should they? So we should worry about their training organisations. Get it yet? Here's a tip: Stay away from a tech agency where the instructors have trouble staying alive. Below I've added commentary to each of the brief reports to help you get some 'use' from them. If you want someone to dig out the coroners' court proceedings and the autopsy reports, compile them and deliver them to you, I'll put you in touch with a net researcher. She can probably even locate pictures of the recovered bodies. But she'll charge you for it. He knew one of his second stages was faulty. He did EVERYTHING he could to survive AFTER he lost his gas. But with that dodgy gear he was dead before he entered the water.The fight for survival was magnificent. The investigators' account is harrowing reading. Ultimately a rescuer almost died with him.
