There are alot of questions concerning what happened, and I don't mean to
disrespect the guy; but from everything I've gleaned from the second hand
information can be summed up into one simple answer..

He was careless.. He rolled the dice and got bite in the ass. There's a
bloody list of things done wrong. And the sad thing is that he probably
knew that he was riding the razor's edge, and didn't give a damn.
